Running Head: Decision Making in a Healthcare Organization 1 Decision Making in a Healthcare Organization Xavier Hunt MHA 601: Principles of Healthcare Administration Professor Jack Lazzare December 19, 2011 DECISION MAKING IN HEALTHCARE ORGANIZATION 2 In the health care profession a CEO will be asked to make a lot of difficult decisions. The CEO will not only have to make decisions that fit the ongoing vision of the healthcare facility he/she may work at they will also have to form stable relationships with their peers, media, and staff. Decision making occurs in all organizations. The decision-making process begins with identifying a question, problem, or area needing improvement, or an operational issue. Problems, issues, questions, and operational challenges come to leaders and managers from many different people both within and outside the health organization (Pablo, 1996). To make the important decision that was given to us in the case study, I would use the garbage can model and its subsets to help me come to a conclusion. This decision involves a lot of people so the choice that will be made will affect all parties involved. In this paper I will discuss how I would tackle this problem with the various tools of decision making through the readings that we have had. The Garbage Can Model How can the garbage can model help in this situation? The garbage can model was invented to expand the understanding of the organization. It addresses the problem that is going on at that time and accepts confusion as a reality. In the garbage can model loose coupling is required in an organization to understand decision making. The garbage can model has also been introduced as a possible method to understand how an organization learns (Tsang, 1997). In the case study the CEO is faced with a myriad of issues like for instance trying to keep the physicians, and adequate staff at the hospital not only that if they were to leave they have threatened to go to their rivals hospitals across the city. The CEO also has to deal with a possible DECISION MAKING IN HEALTHCARE ORGANIZATION 3 lawsuit because the nurse who possibly has HIV has been asked to leave the operating room to work somewhere else. Would this not spark a fire? Could asking the nurse to leave no matter how they tried to butter the deal up still seem like covert discrimination? So the hospital is about to be hit by a media freight train if they cannot get this under control. The garbage can model allows for the CEO to handle these problems accordingly. â€Å"Garbage can decisions can occur in any organization but are more likely to be found in ‘organized anarchies’ where decisions are made under ambiguity and fluid involvement of participants. † Garbage can models are attempts at finding logic and order in the mist of decision-making chaos. Garbage, defined as sets of problems, solutions, energy, and participants, is dumped into a can as they are produced (streams of â€Å"garbage† in time) and when the can is full, a decision is made and removed from the scenario (Takahashi, 2002). The garbage can model depicts the chaotic nature of decision making. Temporal Order Temporal order is a component of the garbage can model. Temporal order replaces sequential order. Time is spatial in that a multitude of issues, problems, information flows, and sensing mechanisms can bombard decision makers in short or long time blocks. How problems and information to resolve the problems arrives in time has relatively equal priority as the evaluation of their importance. Arrival time and sequence in the current context influences decision makers’ attention to the situation. â€Å"The process is thoroughly and generally sensitive to load. An increase in the number of problems, relative to the energy available to work on them, makes problems less likely to be solved, decision makers more likely to shift from one arena to another more frequently, and choices longer to make and less likely to resolve problems† DECISION MAKING IN HEALTHCARE ORGANIZATION 4 (March, 1996). Individuals in the decision-making process, directly and indirectly, are interconnected and influence the context of the decision at hand. The amount of time that the CEO has to make his decision is limited he has to try to make an informed decision about a very sensitive issue that involves a lot of individuals. His decision also will take an effect on peoples family members that use the hospitals services. If he keeps the man will they still want to use the hospital? Or will they go to the cross cities rival hospital? The CEOs problems increased when the media got involved. The CEO has to find a way to balance his time so no one issue become bigger than the other because if he does this something will be forgotten in the process. To combat these issues the CEO will have to be attentive. Attention demands influence decision making. Time and energy must be allocated to understand, evaluate, and formulate a problem, then synthesize relevant information, evaluate options, and finally choose an alternative to counter or terminate the problem. Individuals focus on some things and do not attend to others in the same space of time. Corporate actions, outcomes, and responsiveness are the results of dynamic organizational processes, not heuristics of individual choice (Swanson 1996). Time and energy combine to form â€Å"attention. † Attention is a dynamic concept that is highly dependent on load (that is the number of decisions that need to be made). Information to Media The information that is conveyed to the media is an important tool in this case study. These sites allow us to get information much more quickly, and the way a decision is made and perceived can be misconstrued. Information richness is defined as the ability of information to change understanding within a time interval. The longer the time interval to exchange understanding, the less rich the information. Consequently, the less time required the more rich the information is to the communicators (sender and receiver). The media (such as email, the telephone, or face-to-face conversation) that carries information to intended audiences also has a richness associated with it. A continuum of media richness has been established based on the medium’s capacity for immediate feedback, the number of cues and channels utilized personalization, and language variety. In decreasing media richness, the continuum of richness consists of: â€Å"1) face-to-face, 2) telephone, 3) personal documents such as letters, memos, and emails, 4) impersonal written documents, and 5) numeric documents. † The richer the media, the better equivocality can be reduced; media low in richness is best used when communicating messages that are understood well and possess standard information (Draft, 1986). Simply put, face-to-face interaction works best and bulletin board flyers work the worst in transferring meaning. Leaders and managers must consider what media with which to communicate to ensure the highest probability to transfer meaning to their intended audience. So as CEO he need to make sure that he communicate well with the media saying exactly what he means at all times. My final decision would be to not remove the nurse from the operating room because if he is singled out a lawsuit would ensue and it would be hard to say that he was not removed because it is suspected that he has HIV. As long as everyone followed hospitals protocols such as use of blunt tip sutures as approved by the surgeon, a hands free neutral zone large enough where sharps can be placed and not easily tipped over and the use of double gloves in the operating room if would be okay for everyone to work there. If these procedures were taken lightly whoever did not follow would be reprimanded because there is little room for error in this type of setting. The Success or Failure of Baz Luhrmann’s Movie, The Great Gatsby

Baz Luhrmann’s The Great Gatsby attempts to recreate the novel, but fails miserably. F. Scott Fitzgerald’s The Great Gatsby is very different in meaning from Luhrmann’s filmic adaptation. The film, unlike the novel, makes Gatsby seem like a fool in love, and fails to show the idealism behind Gatsby’s dream. Baz Luhrmann’s The Great Gatsby attempts to recreate F. Scott Fitzgerald’s themes, but ends up failing miserably. The film tried to juggle too many themes at once, rather than focusing a couple crucial ones throughout the movie. The film was unable to convey the failure of the â€Å"American Dream† that Fitzgerald so aptly analyzed throughout his novel. Fitzgerald, unlike Luhrmann, was able to make the reader see that Gatsby’s vision (all of his wealth and grandeur had a purpose: to win over Daisy), which parallels the â€Å"American Dream† that many people strive for. The impossibility of this dream is keyed in on through the dialogue between Nick and Gatsby during Luhrmann’s film, but it does not even come close to achieving the same effect that Fitzgerald did in his novel. Luhrmann focuses on how Gatsby’s hope and determination to repeat the past in this one scene, but fails to tie this in throughout the movie. This important theme is forgotten by the end of the film because of how gaudily the film is stylized. The film may have accurately portrayed the imagery of the extravagant parties that Fitzgerald vividly described, but the glitzy diversions detract from the central themes. The disjointedness of the film, as a result of these distractions, cause the viewer to forget the content of the film. Luhrmann’s attention to the visual and musical aspects of the film prevented the film from exploring the themes that Fitzgerald was able to do by ensuring that the imagery served a greater purpose (not just to become a box office success in Luhrmann’s case). Also, the changes to Nick’s character are significant as it shifts the focus from Gatsby to Nick. Fitzgerald purposefully leaves Nick’s life ambiguous, but Luhrmann attempts to give Nick’s departure from the East an edge by turning him into a drunkard. Owning a home, having a family, and basically living care free is the â€Å"dream† that is projected, but is this goal that so many people are told to strive for even obtainable? There is a belief that once this goal is achieve all the troubles will some how fly out the window, but sadly that isn’t true. The American dream itself is nothing easy to gain, and the quest for this dream can tear anybody that isn’t ready apart. Through Jackson’s â€Å"The Residue Years† and Diaz’s â€Å"This Is How You Lose Her†, they were able to show how difficult it is to achieve the â€Å"dream† and how the journey could affect not only the person striving for it but also the people around them. With both of their unique writings styles they are able to let the reader feel connected with the person on a personal level. Champ from â€Å"The Residue Years† and Ramon from â€Å"This Is How You Lose Her† are two characters that are working towards the American dream, but don’t see how everyone around them is affected by their actions. At times chasing a certain dreams can cause a person to neglect the people and situations around them. The odds are already stacked against anyone who is not white, especially when trying to achieve the American dream or should i say White Dream. The problem with a lot of people are they are trying to obtain something that is not for them, for example the American dream is a belief that has been already thought out for everybody, and if this dream isn’t met then a person is considered unsuccessful. Why chase a dream that was only meant for certain people, like Champ said â€Å"You’re right, not everything’s about race. But what if this is? (Jackson 80)†. Champ is trying to reach this goal and keep his family from falling apart while trying to provide for his girlfriend, mothers, and two little brothers by selling the same drugs his mother is hooked on. He wasn’t good enough at basketball to go pro and doesn’t believe furthering his education will give him what he wants. Champ, an African American male, couldn’t play basketball so he sell drugs, fits the stereotype doesn’t it. Ramon on the other hand, a Latino male, been in America for ten years working at a bread factory trying to achieve the American dream with his wife back home and his mistress in the states supporting him. Champ sees everything he has worked hard for crumbling before his very eyes and it’s all thanks to the family that he wants to save. Ramon on the other hand just wants that house because he believes it’s the gateway to a better life. The woman he is trying to bring into this better life is the same person who doesn’t believe in him, He rubs at his stubble. What if i find the place? You want me to make the decision myself? I dont think it will happen. (Diaz 32). With no one by their sides and their backs against the wall, you just start to feel for these characters. If their loved ones aren’t behind them, then who is? The sad thing about the American dream is not everyone will see it. Ramon hard work paid off, he was able to make his dreams come true, Look, he says holding up the paperwork. Look. He is almost pleading. Im truly happy for him. You did it mi amor. We did it, he says quietly. Now we can begin. Then he puts his head down on the table and cries(Diaz 38). This emotional scene in the book is heartwarming and touching, just to see a hardworking hispanic male have the ability to make his dreams into a reality in American is something that should be applauded. Sadly the same thing can’t be said for Champ. Champ’s hard work and huge heart wasn’t enough,â€Å"He digs into my jeans and lifts the sack into view, my work rocked up and wrapped in clear plastic. They toss up cuffed into their hard back seats and boom the door shut(Jackson 340). Despite everything he tried to do for his family, at the end the person who he was trying to save is the main reason he got arrested. Through both Jackson’s and Diaz’s works they were able to show how difficult chasing the american dreams can be, and make the reader feel for the characters and what they’re going through; and also how hard times can get when trying to pursuit ones true happiest. Free Time Essay There are lots of pleasure grounds all over the world. I think everywhere there are pubs, discoes, clubs, etc. We go out with my friends especially to pubs where we can chat, listen to music, dance and eat something. But the garden parties are more friendlier and cheaper. We usually organize barbecues with children, this time they can play and run in the garden. I usually go to a wellness-weekend with my husband once a year in winter because we work a lot all of the year. There are some well-known spas in Hungary, for example, Hà ©và ­z and Hajdà ºszoboszlà ³, and many people, including foreigners, go there. A hobby is an activity that you like doing and do quite often. Hobbies are a form of recreation. If you have a hobby, it’s easier to regain energy after a hard day’s work. Depending on the type of the hobby, you can also make friends. Unfortunately many Hungarians spend their free time at home watching TV. But there are some who do some physical activities regularly, like swimming, aerobics or hiking. Nowadays many young people spend much of their time playing computer games either at home or in internet cafes. I think men and women usually have different kinds of hobbies, for example, very few women go fishing and they are not so interested in extreme sports, either. On the other hand, men generally aren’t interested in needlework or pottery. In my opinion both men and women do gardening but they tend to do different jobs in the garden. Women like dealing with flowers, they weed the flowerbeds, plant new varieties and they design the whole garden. Men usually prune the trees, trim the hedges and mow the lawn. Collecting stamps, coins, autographs, phone cards, beer mats or napkins, model building, doing needlework or woodwork, pottery, basketry, playing an instrument and dancing are indoor hobbies, while hiking, mountaineering, caving, sailing, fishing, canoeing and bird-watching are outdoor hobbies. My sister’s hobbies are collecting phone cards and diary writing. I like reading about feng-shui and I like to be an Avon consultant. I used to play handball for years, nowadays I only watch handball games on TV. As far as I know, the English like playing golf and cricket, and there are quite a few soccer fans among them. They often mow the lawn and wash their cars at weekends, while some British people go to horse races to bet or just to watch the race. Some Americans go to baseball and basketball games regularly to support their teams. But there are others who prefer staying at home and watching these games on TV while eating snacks. I think Hungarian people collect the same things that people like collecting all over the world, mostly stamps and coins. If we considered a sport that many people pursue popular, the most popular sport in Hungary are football, handball, hiking and aerobics. If a popular sport means a sport that many people watch, besides football and handball, the most popular sports are Formula 1 racing, water polo and boxing.

Social Issue: Sex Work Sex work is a complex social issue that has historical and current underpinnings. It is perceived and performed differently with regards to different periods that construct it. Various perspectives that pertain to violence used, social perceptions, different ways sex is understood while different factors that shape it are examined. Current and future literature and research consider the way sex work is socially constructed and used as commodity. It is such perceptions that shape the way prostitution is performed and the way it influences society. Through methods employed involving research on literature pertaining to sex work further research has been conducted that unveils pertinent factors that shape sex work and its industry. Perceptions on sex as work in literature Literature on sex work pertains mostly to the nature and consequences that is stimulated by political agenda. It has been influenced to a high degree by feminist perceptions that view sex work as a form of male domination. It is not only exhibition of violence but violence itself. There is no forced or voluntary prostitution that is always coerced in certain way even if there is lack of awareness. The motivation behind prostitution has been explained as the expression of hatred towards female body although empirical studies fail to confirm this. Various publications view prostitution as oppression, violence, impinging human rights (Weitzer p. 212, 2005). Males develop attitudes toward violence to exhibit their masculinity in sport, society, and military. Also Western organizations remain predominantly managed by men. Violence can also involve the denial of promise, cancellation of a project, theft, or destruction of something of value (Brewis Linstead p. 22, 2000). Organizational pressure that enforces certain behaviour organized around certain organizing principle so that sentiment, passion, and unpredictability are banished (Brewis Linstead p. 23, 2000). According to Marx and Weber organizations are constructed in such a way as to impose domination, where violence is adopted through authority and command. According to Foucalt, the Ancient Greeks regarded healthy existence as dependent on the way humans engaged in sex. For the Ancients it was the diet that was the main focus of maintaining healthy human existence. The Western culture transformed the way we viewed our bodies, individuality and history. Sexuality changed through different conceptualization and forms in different periods and cultures (Brewis Linstead p. 191, 2000). The male sexual drive has been considered to have been formed through the perception of the male sexual drive as need and drive as part of biology, patriarchal expression of family, where male fidelity is not that important, and where sex is viewed in a permissive way that can incorporate two men and two women (Brewis Linstead p. 191, 2000). Other aspects also involve diversity, social determinants, stigma, and social exclusion on life chances. Critical issues that also concern government interest moves from social inclusion and harm reduction aspects. There is need to reduce harm, such as violence against those working in sex industry. The research deals with them more as one dimensional rather than people with lives of various dimensions. There are different regions, different sectors, and different sellers and buyers and various aspects of sex as work or exploitation (Benoit Shaver, 2006). Prostitutes that work on streets have been found to suffer greater exposure than those that work in massage parlours due to drug use. They suffer greater health problems, vein thrombosis, chest infections, hepatitis B and C, anxiety, and depression. Sex workers in sex parlours suffered only half as much as those that worked on streets and had different health needs (Anonymous, 2007). Media has been found to contribute to the construction, reproduction, and social stigmas associated with sex industry. The complexity of issues involving sex work pertains to the diversity of people that are part of the industry, where different ways of participation and diverse relationships are formed. Various perspectives incorporate prostitution and its exploitative aspect in terms of work, slavery, and freedom. Previous research may have included certain flaws in the way it was conducted through standardization. Social-legal environments may encourage sex work access to resources (Benoit Shaver, 2006). A research in Netherlands showed the level of burnout reached three dimensions involving emotional exhaustion, depersonalization, and personal competence. The reasons given included lack of choice, negative social interactions including violence, and lack of control in interaction with clients. Depersonalization was used to handle interactions with clients and was part of handling negative experiences. High level of emotional stress led to depression, anxiety, and hostility. Coercion in sex industry Prostitution as part of psychological repression is difficult to ascertain as it can stem from ideology or individual features. Desire and need for certain type of experience can stimulate the demand for such an experience (Brewis Linstead p. 196, 2000). Even though prostitution reflects certain imbalance, it is unlikely to disappear along with the disappearance of such imbalance. Freud implicated desire as the desire for the desire of another person rather than the desire of such person only. Such perspective incorporates a symbolic aspect rather than the expression of domination. According to Lyotard, there are two forms of desire, where its central aspect is wish and when it is based on lack and represents coercion. In a similar way to Freud, Lyotard also perceived desire as the outcome of energy that is part of psychic means leading to the expression of energy flow. Along with Deleuze he conceives of postmodern desire as part of sensation rather than signification (Brewis Linstead p. 199, 2000). For Lyotard, through the fragmentation of desire the experience can be intensified along with authorization of existence (Brewis Linstead, p. 203). As emotions become part of pleasure leading to imaginative hedonism expressed through modern consumption. For Baudrillard, modern consumerist society adopted hedonistic morality based on pleasure in place of previous puritan morality (Brewis Linstead p. 208, 2000). Drug use is a way of coping with sex work. Under the influence of drugs, workers can alter their conception of their own self, adopting also a different identity. Hard drugs can also numb the awareness of the act. Through the use of drugs the work itself can be handled better along with better management of safer sex negotiation (Brewis Linstead p. 212, 2000). Another manner of coping involves psychological barrier. For a prostitute, work involves selling herself rather than just the product. Selling skills involves also certain right to the person of the worker during employment in a similar way to gaining control of the body when having sex. Prostitution incorporates the person and the body as opposed to profits that result from certain activities executed by employees although prostitutes receive money for the use of their body in a more real way (Brewis Linstead p. 227, 2000). Some prostitutes experience victimization through the lack of control over their bodies and what is being done to them. They feel anger due to their powerlessness that often can be result of coercion through those who are close to them, such as their boyfriends who are pimps. Threats that drive women on the streets encourage fear, forcing them to follow what they are told (Brewis Linstead p. 228, 2000). According to Vanwesenbeeck (2001), the reasons behind engaging in sex work stemmed from early victimization as part of the literature in the 20th century, where a high number of prostitutes were victims of abuse. Childhood abuse and prostitution were part of coping behaviour as part of stress work while others indicated that stagmatization along with various factors that involve institutionalization, association with pimps, drug abuse, and low employment possibilities led to engaging in prostitution. Childhood sexual abuse and prostitution were also linked through the above factors. Sexual victimization has been connected with prostitution in the Western world. Physical and sexual abuse, difficult family upbringing, and sexual precocity encouraged engaging in sex work. Lack of family attachment in females led to the association with older and a larger number of partners. Also running away would lead to prostitution. Such backgrounds would facilitate engaging in sex work. Sexual victimization during childhood was a higher determinant behind getting into prostitution than running away that was higher even than drug use. In non-Western countries it is economic situation that forces engaging in sex work rather than abuse. In some Taiwanese families the sense of obligation forced certain women to engage in prostitution or debt repayments, lifestyle satisfaction, while fraud or force constituted only a small proportion. There has also been migration as part of prostitution although due to lack of documented research it is difficult to estimate the actual number of those that engage in prostitution in this way (Vanwesenbeeck, 2001). Economic necessity along with sex work illegality enables traffickers to exploit the situation although there is insufficient data that can provide insights on patterns and exact numbers. Research concerning victimization data on the part of prostitutes indicates that sex work is traumatizing (Vanwesenbeeck, 2001). Motivating factors Literature pertaining to motives behind seeking sexual activities differ at different time periods. Before 1990, sexual activity was desired because of its variety and freedom to experiment while certain features, such as lack of emotional involvement seemed attractive. There have been less shame, pregnancy fear, more mystery, excitement while also being part of ego-boosting made it attractive. In Australia clients were less socially effective and sensation seeking while for Dutch men sexual sensation was most desirable among those seeking transsexual and transvestite sex. In New Zealand, relaxation, reluctance to engage in game playing and obligations were behind engaging in commercial sex (Vanwesenbeeck, 2001). Sex work perceptions Only a small number of American population perceived prostitution as legal. They included mostly Catholics, men, and Whites. Western feminists attach stigma to sex workers while receiving a paradoxical view due to its different attitude as work. As part of structural economic inequalities, improvements in the way sex work is treated are limited. Such improvements as part of social stigma attached are impeded. Through the illegal status and social stigma sex workers are forced to endure the treatment they receive. It is through gender discrimination hence that adequate policies cannot be e ffected (Vanwesenbeeck, 2001). While Lyotard views sex work as an inner expression of certain unfulfilled desires that are manipulated by society itself, Vanwesenbeeck perceives is legal status and socially constructed behaviours responsible for the way prostitution is effected. The lack of acceptance of sex and race that are viewed negatively leads to the lack of acceptance also in the public life. According to Morgan, cited by Sojourner, it is the lack of acceptance of desires and their perception as troublesome that allows for the ability to be controlled. In this way society can impose certain wants and desires, where society accepts certain desires as its own even though they are exerted externally (Sojourner, 1988). Future research According to Weitzer, inadequate findings so far result from the concentration of research on street sex workers where different factors are involved as part of such work. Less research has been conducted in relation to indoor prostitution, male and female clients, transgender workers, and managers. Addressing such perspective can lead to a better view on work experience, power relations involved, gender inequality as part of different types of prostitution. It would also provide a more balanced view than the radical feminist view that is one-dimensional (Weitzer, p. 230). Management of prostitution exercised those who control workers while deriving profit from them. Although pimps most frequently promote prostitution and have almost complete control of their workers their protection is often viewed as inadequate while they themselves engage in rape and assaults. A small minority of sex workers admitted that their pimps offered them protection while engaging most frequently with fighting other pimps and competition. Different practices employed different pimps, in the same way that different establishments treat differently their sex workers (Weitzer, p. 228). Sex at work poses problems pertaining to sexual harassment or gender discrimination. There can be various manners and means employed that can deal with these issues, such as admitting any affairs taking place at work, where consensual agreements are reached and pertinent rules can be specified (Brewis Linstead, 2000). Conclusion There are various factors responsible for the way sex work influences society. Apart from motivating factors that drive participation, different means of coercion transform the way sex work is conducted. Viewed previously as the expression of prohibited desire, prostitution evolved into commodification of such desires. Socially constructed perceptions of sex work at the same time stimulate its participation. Although the radical feminist view perceives sex work as the expression of male domination, there are also other pertinent issues that are part of complex issues as well as relationship, including power relations involved. Sex coercion as part of human trafficking is another outcome of the social dilemma that involves sex work, constituting a serious although insufficiently documented issue. The number of children being home schooled increased from 32,804 in 2000 to 53,216 in 2003 (Brown 1). There are three basic reasons parents keeps their children out of public schools.
